Before your scheduled appointment, please fill out “WVELC Patient Registration Forms”, “WVELC Vision Insurance Form”, and “COVID Questionnaire” and bring them in these form with you before your next visit. 

If you are unable to print or fill out your forms ahead of time, please give us a call and let us know. 

We also have included a copy of our WVELC Privacy Practice Policy. Our sister clinic, “Bellevue Eye Specialists”, shares the same Privacy Practice Policy as us. We are dedicated to providing you not only the best care, but protecting your private information. If you have any questions regarding our privacy policies, please reach out to us.
